    Care is for adults 18+ who are physically located in Florida during each telehealth visit. Review clinical fit, availability, insurance participation, and expected fees before arranging care.

    Discuss an appointment with Kizito L. Mbaekwe, MSN, APRN, PMHNP-BC, the practice’s sole provider. Follow the instructions supplied directly by the practice or your referring clinician; appointments are confirmed by the practice.

  3. Step 3: Complete Intake

    Use the practice’s designated secure intake system to complete your health history, consent documents, and any requested screening questionnaires. Do not upload or email these through the public website.

  4. Step 4: Attend Appointment

    Join from a quiet, private location using the video link in your appointment instructions. Have your medication list and questions ready. Your provider will confirm your identity and physical location.

    Discuss the assessment and decide on appropriate next steps together. Your plan may include medication, therapy referrals, practical supports, or additional evaluation. An appointment does not guarantee a prescription.

Before you commit to an appointment, you deserve to understand your expected costs. Insurance participation and self-pay rates must be confirmed with the practice; no plan is presumed to be in-network.

Confirm the full picture before booking
Ask about the visit fee, payment timing, cancellation policy, and any separate laboratory, pharmacy, or outside-provider charges. If using insurance, confirm network status, deductibles, copays, coinsurance, and telehealth benefits with the practice and your plan. Reimbursement is not guaranteed.

No Surprises Act notice

You have the right to receive a Good Faith Estimate.

Under federal law, if you do not have health insurance or are not using it to pay for care, you generally have the right to a written estimate of expected charges for non-emergency health care. You can request an estimate before scheduling.

  • Providers generally must give an estimate when care is scheduled at least 3 business days in advance, or when you ask for one. The delivery deadline depends on when you schedule or request it.
  • Ask each involved provider or facility for an estimate of their expected charges. An estimate is not a bill, and unanticipated care may not be included.
  • If a provider bills you at least $400 more than their estimate, you may be eligible for the federal patient–provider dispute resolution process. Start a dispute within 120 calendar days of the initial bill.
  • Keep a copy or photograph of your written estimate and compare it with your bill.

You must be physically located in Florida at the time of each appointment. Travel can affect eligibility, even if your home address has not changed. Confirm your location before booking or traveling. Telehealth may not be appropriate for every concern, and an in-person assessment or referral may be recommended.

    Find a quiet, well-lit space where you can speak freely. Headphones can help. Do not join while driving, in transit, or in a public setting.

  4. Step 4: Join through your verified link

    Open the appointment link a few minutes early. Allow camera and microphone access on the visit platform, and use its device check if one is available.
